Many of us have never thought about the care we would want toward the end of our lives or if something should happen. However, it’s something we should all do.

On __ (insert date) ___  we will be hosting an information booth in (insert location) from (insert time) to share information about how you can have these important conversations.  We will also have copies of Alabama’s official forms for stating your wishes, along with a helpful booklet with all types of questions to help think through the decisions.

We hope you will stop by our booth, pick up some materials and ask any questions.

On __ (insert date, time and location) ___  we will be hosting a meeting to share information about how you can have these important conversations.  We will also have copies of Alabama’s official forms for stating your wishes, along with a terrific booklet with all types of questions to help think through the decisions.  The meeting is free and is being hosted by _________.

We hope you will consider attending and that you’ll bring a family member or friend with you.  We plan for everything else.  Why not document our wishes for care through the end of life, so those wishes can be understood and respected?
